About this questionnaire

The WHO-5 is one of the most widely used wellbeing questionnaires. All five statements are positive, so it measures the presence of wellbeing rather than symptoms.

For each statement, choose how much of the time it has been true for you over the last two weeks.

Scoring: 0–5 per statement, added up and multiplied by 4 to give 0–100. Higher is better.

Before you start

For reflection, not diagnosis. This is a self-report questionnaire. It cannot diagnose any condition, and it is not a basis for hiring, admission or any other decision about a person.

Private by default. Everything runs in your browser. Your answers are not saved or sent anywhere unless you choose, before starting, to share them anonymously for site statistics.

Provided as is. A personal project, free and with no warranty. Not connected to any institution I work with.
