Mood check (PHQ-9)
Nine questions about the last two weeks, the same ones many doctors use as a first check for depression.
Your answers stay in this browser tab unless you choose to share them anonymously.
About this questionnaire
The Patient Health Questionnaire-9 asks how often you have been bothered by the nine symptoms used to recognise depression. It is short, well validated and used in clinics around the world.
Think about the last two weeks and choose how often each problem has bothered you. A final question asks how much these problems affect daily life.
Scoring: 0–3 per question, total 0–27. 5, 10, 15 and 20 mark mild, moderate, moderately severe and severe.
Before you start
This includes a question about thoughts of death or self-harm. If those thoughts are with you now, please contact someone today: in Japan よりそいホットライン 0120-279-338 (24 hours), in Korea 109, in Indonesia 119 ext. 8, elsewhere findahelpline.com, or your local emergency number.
For reflection, not diagnosis. This is a self-report questionnaire. It cannot diagnose any condition, and it is not a basis for hiring, admission or any other decision about a person.
Private by default. Everything runs in your browser. Your answers are not saved or sent anywhere unless you choose, before starting, to share them anonymously for site statistics.
Provided as is. A personal project, free and with no warranty. Not connected to any institution I work with.
